Online Basic Will Application Form

Fill out the details below for us to begin to draft a new Will for you.

Please note that this form is used for basic Wills only and only for clients who are able to come in and sign their Wills at our office in Mount Eliza. If you require a testamentary trust or a complex Will please contact us to arrange an appointment to meet with us to discuss your needs.

The form below collects the information we need to draft a Will on your behalf. Please make sure you answer all questions accurately and leave any extra information in the comments box below.

About You






Executor(s)
Things to consider:
Make sure you have asked the proposed Executor if they are willing to act for you before appointing them.
They need to be over 18 and preferably under 70 and have the ability to carry out the duties of an Executor.

The Executor is responsible for seeing that the terms of the will are carried out and for applying for probate. In most circumstances, a husband and wife will appoint each other to act as Executor.
Please note that you may have more than one executor or alternate executor and more than this form provides for, if you wish to have more than two please make note of it at the end of this form.

An alternate executor is someone you nominate in the event that your nominated executor is unable or unwilling to act. We recommend having at least one alternate executor.

Your Assets/Beneficiaries
Things to consider:

Most married couples leave their entire estate to the surviving partner. If the partner has already passed away then the estate would usually go to children, grandchildren, parents, siblings, relatives or charities.

If you have specific items or assets you wish to specific people then you can keep a separate list of these with your Will.


Please list the full name, address and relationship to you of each intended beneficiary.

Prior Will(s)


Keeping the Will:

We recommend that you keep the original with your personal papers. We note that we will hold a copy of your Will on our file should it be required.

To make it easier on your Executor you might wish to have a list of your assets and a list of people and organisations to be informed of your death kept with your Will.
